So. We've established that one bad night of sleep reduces insulin sensitivity by 21% in people with T1D.
Now let me tell you what happens when a person without diabetes has a bad night of sleep.
Their insulin sensitivity drops too. Same basic mechanism. Sleep deprivation affects insulin response in all humans. It's not a T1D-specific thing.
Here's the difference.
Their pancreas notices. And it makes more insulin to compensate.

All right. Welcome back. We're in Week 2 of the Sleep Challenge. Monday we talked about the 21% insulin sensitivity reduction from one bad night of sleep. Today I want to go one layer deeper on why that hits us differently than it hits everyone else.
Because the 21% effect is not exclusive to T1D. Sleep deprivation reduces insulin sensitivity in all humans. It is a universal biological response.
The difference is what happens next.
In a person without diabetes, the system has a feedback loop. Insulin sensitivity drops. Blood sugar starts to tick up slightly. The pancreas detects this, spits out more insulin to cover it, and the whole situation resolves quietly before that person is even aware it happened. It's a SITUATION!!
They wake up, make coffee, go about their day. Their body just handled it. Overnight. Automatically. Without waking them up or requiring any intervention.
That is a remarkable thing that a healthy pancreas does. And I say this as someone whose pancreas got picked last in dodgeball back in 1992. Valentine's Day, specifically. My pancreas was the last one standing on the gymnasium wall...just begging to get picked.
In a T1D body, that feedback loop does not exist. The pancreas detects zippo and did nothing, because the pancreas is air quotes..."not available". The insulin sensitivity dropped, the blood sugar climbed, and nothing automatically compensated for it. The penalty just lands.
That's the whole difference. One group has a biological safety net. The other group's safety net is gone.
And so the 21% that a non-diabetic person's body quietly handles before they wake up? That becomes a blood sugar problem that we manage manually. We correct for it. We figure it out. We do the extra work. But it costs something. It costs correction doses, and mental energy, and time, and a lotta times it puts us in a spot where we're running higher for part of the morning and wondering what we did wrong.
We didn't do anything wrong. We just woke up without a backup system.
Now. I want to be really clear about something. I'm not saying this to be discouraging. I'm saying it because it matters for how you understand your own management.
For most people, sleep is a health recommendation. Get your eight hours, it's good for you, sure, fine, we'll try. Thanks grandma... I think this is why my grandparents used to go to bed at like 7PM and get up at 4AM. But my grandparents ate Spam...so, there's no accounting for that.
For us, sleep is more than a health recommendation. It's a metabolic variable that directly affects how our primary treatment works. That puts it in a different category.
And if you've ever had the experience of waking up higher than you went to bed on a night where nothing unusual happened, let me tell you about that too. Because there's a very specific, documented reason why T1D bodies tend to run higher overnight, and it has nothing to do with what you ate, and it involves your liver doing something at 3am that you did not authorize.
We're talking about that on Friday.
But right now, before we get there, I want to name something.
You've been managing this disease in conditions that are structurally harder than they are for people without T1D. Not dramatically. Not in a way that says "here I am and my life is sooooooo terrible". It just announces itself. Just quietly, daily, there's no backup system and you're doing the backup system's job manually.
You've been doing the job of a pancreas that isn't there. And you've been doing it half asleep, or trying to silence all those alarms, or running low at 2am, or just lying there at 3am doing diabetes math in your head.
That's a lot of work. I think it deserves to be said out loud.
You have been doing the job that another body part was supposed to do. And it fell asleep on the job.
You've been doing it without the full picture. Now you have a little more of it.
You're not doing a bad job. You're doing a difficult job and I'm proud of you.
